Answer: A. Sterile gloves must be worn during the procedure. 

Currently, research is insufficient to support routine use of sterile gloves during implanted port accessing and deaccessing procedures. Answer B is recommended because it is important to palpate the area around the port (https://www.ons.org/store/books/access-device-standards-practice-oncology-nursing) for warmth and tenderness and to determine the orientation of the port prior to access.   

Answer C is recommended because ensuring adequate blood return prior to use of the port (https://www.ons.org/store/books/access-device-standards-practice-oncology-nursing) can help reduce the incidence of infiltration and indicate proper needle insertion.  

Answer D is recommended because use of a noncoring needle separates the silicone septum of the port (https://www.ons.org/store/books/access-device-standards-practice-oncology-nursing) during accessing and prevents coring the material, which could lead to debris in the port reservoir and leakage.
